Swan Falls Assisted Living provides a diverse selection of daily activities, including arts and crafts, holiday festivities, birthday parties, bingo, and other events, for its residents. The atmosphere and activities are designed to encourage residents to socialize and engage with others. Residents can also take advantage of the elegantly landscaped grounds, featuring gazebos for relaxation and socializing. Cozy sitting areas and a sunny living room are available for rest, and the dining room offers three nutritious meals a day.

Memory Care 

For residents in need of memory care, our person-centered THRIVE program offers engaging roles and life activities to promote positive outcomes. Individualized daily programs are tailored to reinforce and maximize each resident's remaining strengths and abilities. Family members and loved ones are encouraged to visit and take part in the active lives of residents.
