Swan Falls is a community located in Kuna, Idaho that provides assisted living and memory care services to its residents. Unlike traditional facilities, we have designed our community with several small buildings instead of one large space, which enables us to offer residents personalized attention and care. Our goal is to create an elegant, comfortable, and hygienic environment that supports resident well-being with empathy.
We offer various services, including aid with daily activities and a specialized memory care unit, to assist residents in maintaining their independence and enhancing their quality of life. Our team is trained to collaborate with each resident and their family to develop a customized care plan that meets their needs, whether it is support with medication, dressing, and bathing, or the use of our THRIVE memory care services. Additionally, our licensed nursing staff is available round-the-clock to supervise and ensure the safety of our residents, giving you and your family peace of mind.
